 — eClinicalWorks®, a leader in ambulatory healthcare IT solutions, today announces that New Jersey’s NEPA ACO Company LLC selects eClinicalWorks Care Coordination Medical Record® (CCMR), for its more than 80 providers across several practices. The relationship between NEPA ACO and eClinicalWorks will assist the physician-led ACO in efficiently administering the organization and meeting its quality of care goals.

“NEPA ACO focuses on providing its patients with the most passionate care,” said Nader Moaven MD, CEO of NEPA ACO Company LLC. “By utilizing the eClinicalWorks CCMR platform, we have a great opportunity to advance our outcomes with innovative care delivery and payment models by tapping into technology. This population health management platform will give us more insight into our patient populations and the ability to streamline care coordination across providers. eClinicalWorks CCMR will help us increase our savings while also improving the quality of care, creating a successful Accountable Care Organization. We intend to expand our ACO's scope by signing on additional payor contracts and involving more practices in Northern New Jersey. As we do this, the eClinicalWorks platform will be a good support.”

eClinicalWorks CCMR will provide NEPA ACO with visibility into a patient’s care across all settings, facilitate smooth transitions of care, engage patients and providers in preventative care, improve outcomes among populations and reduce costs. A vendor-neutral, open network securely connects ambulatory EHRs with hospital in-patient systems and payer claim feeds, including CMS, to provide cost and utilization for services delivered to patients. Recently recognized by Frost & Sullivan with its 2014 North America Frost & Sullivan Award for Enabling Technology Leadership, eClinicalWorks CCMR will work with NEPA ACO in meeting its goals as an organization.

“eClinicalWorks CCMR assists NEPA ACO in bringing together independent physician groups to increase the quality of healthcare,” said Girish Navani, CEO and co-founder of eClinicalWorks. “This platform gives providers a deeper insight and understanding of patient populations, while discovering the opportunities and challenges that are actionable. We look forward to working with NEPA ACO.”

Included in CCMR, eClinicalWorks Population Health Management Analytics uses closed-loop analytics to deliver reporting, alerting and messaging capabilities necessary to manage population health. Dashboards can be shown at the community level to determine and monitor adherence of clinical process and outcomes, including medication recalls and chronic disease management. Care coordination and analytics work together for a collaborative care framework.
