eClinicalWorks Spotlight on Smart Forms

eClinicalWorks Smart Forms are an extremely useful feature of the eClinicalWorks EHR. In this blog post we'll dive into Smart Forms and what they are, how they're used, and provide a few videos to help you better understand and get started with eCW Smart Forms.

Smart Forms are designed to measure the severity of selected conditions and can be extremely helpful. When a Smart Form is completed, the information populates as structured data in the progress note.

Most smart forms present questions in which the answers determine the options for other questions. This type of built-in intelligence makes smart forms extremely useful when measuring the severity of selected conditions. The smart form may then calculate a score or severity of a condition based on the intelligence built into the form.

Smart forms can be launched from a number of different locations inside eClinicalWorks. You can launch a smart form from the SF dropdown menu, from the ICW under the CDSS alerts tab, and from the social history and HPI sections of the progress note.

Smart forms can be faxed or printed and eCW comes with prebuilt smart forms for things like:

  • Respiratory Illness Screening
  • Smoking/Tobacco control
  • Asthma Severity
  • Alcohol Use or Abuse
  • Patient Mental Health

In addition to the pre-built smart forms, you can also create your own smart forms. Smart forms are often used to measure the severity of selected conditions but can also be used to collect other information from patients via the patient portal.
